Pancreatitis aguda secundaria a hipertrigliceridemia durante el embarazo, reporte de un caso clínico

El caso corresponde a una mujer de 36 años con 35 semanas de gestación, con antecedente de diabetes mellitus tipo 2, hipotiroidismo e hipertrigliceridemia, consultó inicialmente por dolor abdominal, vomito, paraclínicos con leucocitosis y reactantes de fase aguda elevados, ecografía abdominal normal. Inicialmente se sospechó apendicitis aguda, se llevó a cirugía donde se encontró abundante líquido no fétido y membranas fibrinopurulentas en íleon y colon, la apendicitis no se evidenció durante el procedimiento ni en el reporte de la patología. Paraclínicos del postoperatorio con hipertrigliceridemia >1000 mg/dl, amilasa 4 veces mayor de lo normal, cultivos negativos, compatibles con pancreatitis; el tratamiento fue con antibiótico, anticoagulante y líquidos; luego de 7 días presentó preeclampsia severa y se decidió llevar a cesárea, con feto vivo que requirió UCIN (Unidad de Cuidados Intensivos Neonatal) por sepsis y SDR (Síndrome de dificultad Respiratoria), la paciente y el recién nacido progresaron satisfactoriamente sin complicaciones. Los casos de pancreatitis en gestantes son pocos a nivel mundial pero los factores de riesgo están relacionados con los antecedentes que tenía la paciente, raras veces está asociado a preeclampsia incrementando el peligro para vida de la paciente y el feto, es importante conocer más esta patología en las gestantes para evitar mortalidad y realizar diagnóstico oportuno. The case corresponds to a 36 year old woman with 35 weeks of gestation, with a medical history of diabetes mellitus type 2, hypothyroidism and hypertriglyceridemia, she initially consulted for abdominal pain, vomiting, paraclinic with leukocytosis and elevated acute phase reactants, normal abdominal ultrasound. Initially acute appendicitis was suspected and surgery was performed where abundant non-fetid fluid and fibrinopurulent membranes were found in the ileum and colon, appendicitis was not evident during the procedure or in the pathology report. Postoperative paraclinics with hypertriglyceridemia > 1000 mg/dl, amylase 4 times greater than normal, negative cultures and compatible with pancreatitis; The treatment was with antibiotic, anticoagulant and liquids, after 7 days presented severe pre-eclampsia and it was decided to carry a cesarean section, with a live fetus that required NICU (Neonatal Intensive Care Unit) due to sepsis and RDS (Respiratory Distress Syndrome), the patient and the newborn progressed satisfactorily without complications. The cases of pancreatitis in pregnant women are few in the world but the risk factors are related to the antecedents that the patient had, rarely it is associated with pre-eclampsia increasing the danger for life of the patient and the fetus, it is important to know more this pathology in pregnant women to avoid mortality and to perform timely diagnosis.
